MyVaxIndiana Update: After 09/16/2024, Hoosiers can fill an authorization form to update their and/or their minor(s) record with their cellphone number and email on the MyVaxIndiana Web Portal. This is a onetime effort. Now, they can access their or their minor(s) immunization record by validating their PIN or the six digit authorization code sent to their registered cellphone or email.
